Admission/Service Initation Policy & Procedure

The determination to proceed with admission will be based upon the ability of the program to meet the needs and preferences of each person and his or her personal goals consistent with the principles of least restrictive environment, self-determination and recognizing each consumer’s history, dignity, and cultural background: affirming and protecting the consumer’s rights. 

A pre-admission interview will be held to determine the needs of the applicant, considering if this placement will meet the applicant’s needs, and to determine if he/she is interested in being a consumer. 

Admission Criteria
Lifetime Opportunities provides the following residential services: Adult Foster Care, Child Foster Care, Intermediate Care Facility for Persons with Mental Retardation (ICF/MR), Crisis Respite Services and Supported Living Services (SLS)

  1. Adult Foster Care
    1. Persons must have been screened and authorized for services for mental retardation or related conditions.
    2. The determination to admit (or not to admit) an individual will be based upon the program’s ability to meet each consumer’s needs.
    3. The adult sites 18 years of age or older unless the site has a variance or dual license. The child foster care sites can provide services to children six years of age or older, unless the site has a variance or dual license.
  1. Child Foster Care Variations: Is age 6 or older, unless the site has a variance.
  1. ICF/MR Variations
    1. May accept or provide lodging to any who meet Health Department Regulations: 
  1. Crisis Variations
    1. The program will inform the consumer or legal representative of the policy on the limited timelines of the crisis services prior to or the day services are initiated, or sent out by mail within one business day of service initiation.
    2. The license holder has a written policy and procedure on the time limits and parameters of crisis services.
  1. SLS Variations
  2. Lifetime Opportunities also provides Supported Living Services (SLS) to individuals living in an independent placement that require some services but not 24 hour staffing. The schedule for staff is based off the needs for the individual receiving services and may vary from 7 days per week to one day every other week. Staff provide programming based of the individual service plan and can include medication administration to money management.

Service Costs
Daily rates for placement are based off the needs for each individual.   Room and board rates are set by the State of Minnesota and vary for adults or children.
